LEA Registration Form

Use this form to register your entire LEA for NCASA membership. In addition, please print and mail LEA Membership Form Part II with your payment.

Each school must still complete the school registration form. Please notify your high schools that you have registered the LEA and ask them to complete the school registration form. LEA's that register all schools which contain students in grades 9, 10, 11, and / or 12 will receive a 25% discount on the total membership fee. An LEA must contain more than one high school to receive the 25% discount. The school membership fee is $100 per school plus $0.25 per student enrolled in grades 9-12 on the tenth day of school.
